Restraining Order

How do you file a restraining order in NJ?
What about a harassment complaint? Is there any type of evidence needed for either?

I only suspect that someone may have been following me for a time. I have never received anything threatening from this person and was never physically harmed. I received an email via facebook that I thought was phony and could have been from this person but have never received any other correspondence. Someone called the place where I work and asked for me saying they wanted to deliver flowers but I know that was definitely phony. I did have my workplace on FB and I have since made my account more private. I did notice that this person friended a friend of a friend of mine on facebook recently but that could be coincidental. One day, it seemed like someone was watching my house and I called the police. But again, I have no proof that it was this person. So, I don't really have anything concrete. I sent this person an e-mail myself, anonymously, implying that I wanted a relationship to see if I'd get a response and I did not receive a reply. It's someone I think may have some sort of romantic interest but seems like a stalker. I don't think this person will initiate contact with me but may be trying to get info on me online. I was actually in some college classes with this person.
